Long-Term Care Ombudsman

Join a nationally accredited senior center and make a meaningful difference in the lives of older adults across Philadelphia. We are seeking a compassionate, motivated professional to serve as a Long-Term Care Ombudsman, advocating for residents in skilled nursing facilities, personal care homes, domiciliary care homes, and older adult daily living centers.

As a Long-Term Care Ombudsman, you will:
Visit assigned long-term care facilities to build relationships with residents, families, and facility staff.
Serve as a trusted advocate and liaison, helping residents to understand their rights and access available community resources.
Investigate and resolve complaints related to resident care, safety, rights, or service quality.
Collaborate with residents, families, and facility staff to ensure concerns are addressed and resolved effectively.
Maintain accurate, confidential documentation of complaints, actions taken, and outcomes.
Support resident councils and family groups to help empower residents and encourage active engagement in their care.
Represent resident interests during meetings with facilities and state agencies.
Support community outreach efforts to recruit volunteers and educate community members about the Ombudsman program.
Spend one day per week staffing the Ombudsman Intake Line, fielding calls and compiling the preliminary information needed to address a resident’s concerns.
Perform other related duties as needed to support the program’s mission.
To be successful in this role, you will need:
A Bachelor’s degree (required)
A valid PA driver’s license and daily access to a personal vehicle (mileage reimbursement provided)
Strong communication, problem-solving, and interpersonal skills
A commitment to fairness, advocacy, and resident empowerment
The ability to maintain confidentiality and adhere to program regulations
Motivation and organization to thrive while mostly working remotely
This is a primarily field-based role, with staff working remotely and regularly conducting in-person visits to their assigned roster of long-term care facilities. Each Long-Term Care Ombudsman is scheduled to spend one day per week in-person at Center in the Park (5818 Germantown Ave) to manage the Ombudsman Intake Line.
